What We Offer

Our professional carers are recruited in line with our values and are passionate about empowering people to live better lives in their own home. Our visits will be arranged to fit into your existing routine,and offer a wide range of care and support, including:

  • All aspects of personal care, carried out in line with privacy and dignity standards and respecting your preferences at all times.
  • Medication administration and ordering - taking away the stress of managing your meds, and backed up by technology, including eMARs (Electronic Medication Administration Records), for increased efficiency and ease.
  • Hydration and nutrition support, keeping you feeling well.
  • Daily health reviews, recorded to track your progress and flag issues before they become serious.
  • Cera Family App enabling accessible communication - available to you and your family, and any linked professionals (with your consent), enabling peace of mind with access to your visit schedule, visit notes, and health status.
  • Enabling access to the local community, building friendships and relationships outside the home. We work closely with local services and community groups to help you access services that match your interests.
  • Emotional support - hear from our customers about the support they receive or read this letter about the support we offered one family as their loved one came to the end of their life.
  • Nurse-led care, supporting a range of more complex health needs, such as PEG feeds, Oxygen, and Stoma Bags.

Your health and wellbeing is our priority, and our in-house, innovative technology works to ‘Predict and Prevent’ health issues, ensuring early intervention and reducing the likelihood of a serious health issue and/or hospital stay.

Our ‘Predict and Prevent’ technology includes:

  • Our Healthier Lives technology, which uses data collected from your visits and machine-learning AI and algorithms to predict and prevent negative outcomes. This can predict your risk of hospitalisation 7 days in advance with 80% accuracy, and help reduce high-concern issues by 60%+ in 6 weeks.
  • Our Falls Prevention technology, powered by predictive AI and informed through our Enhanced Falls Risk Assessment, proactively assesses your risk of falling by analysing your daily care data and provides targeted interventions. We have achieved a 20% reduction in fall-related incidents for our clients.

Person-centred care

Care at Home for Henry

“Henry”, is a 78-year-old gentleman who was diagnosed with a progressive neurological disorder (similar to Parkinson’s) following a series of falls.

Due to his declining health, it was suggested to Henry that he may want to move into a full-time care setting, as he had no family to help with his care. Henry was distressed at this idea, as he did not want to leave his home.

We called a meeting on behalf of Henry with his allocated Social Worker and Neurological Specialist to assess his future care needs, and whether these could be met at home. In line with Henry’s wishes, this was agreed.

Henry now accesses the local community weekly, through an arrangement with the local taxi firm. Assisted by the same driver each week Henry is able to maintain his independence, while staying safe.

With our care, and help establishing community links, Henry has been able to continue living at home, retain his independence, and live well.
