Designed to empower our customers (with or without a Care Act 2014 Assessment) to take the first steps of reablement, we can offer:

  1. Same-day person-centred, reablement-focused assessment of need, in conjunction with information gathered from local Discharge Teams.
  2. Same-day commencement of tailored care packages.
  3. Short and longer-term, flexible services, providing care and support from a few days to a few weeks.

  • Our recruitment of Carers with demonstrable experience in supporting a wide range of customers.
  • Tailored reablement-focused and recovery-based training for all staff.
  • Constant visibility of staff capacity, through our SmartCare Platform, allowing us to track and plan packages of care in real-time.
  • Bespoke monitoring and real-time reporting, to demonstrate use of hours and efficiency of the service.
  • Our collaborative, multidisciplinary approach to service delivery, based on our years of experience working with Discharge Teams.
  • The co-production of short-term outcomes with the customer, tracked through our Outcomes Tool, designed to demonstrate and chart progress at each visit/daily.
  • Our daily Health Overview (completed by Carers on our Cera Carer App), which provides opportunities to identify post-discharge health and wellbeing issues early, reducing the likelihood of a customer returning to hospital.
  • Our easy-to-use, in-house designed Family App, which (with consent) can give the customer, their family, and key involved professionals/onward progression teams oversight of the customer’s visit notes, progress, and health after each visit.
  • Our policy of positive risk taking, in line with the customer’s comfortable level of risk and our duty of care. We ensure that people are risk-enabled, to challenge themselves and (re)gain skills and confidence.

Our understanding and knowledge of Tech Enabled Care and Assistive Technology, supported by our roll-out of Trusted Assessor training in line with Local Authority support. Our staff can identify adaptations and technologies which can facilitate confidence and reassurance in the customer’s home and beyond, boosting their independence.
