About the Role
As a Mobilisation coordinator you will be responsible for supporting all Complex Care Branches with operational oversight of new packages of care. You will play an integral part in fostering new relationships both internally and externally with all stakeholders for a smooth transition of care from our implementation team to Branch Operations.
Responsibilities
To Support operational processes for the implementation of all new care Packages
To develop and maintain effective and productive relationships with all Internal and external Stakeholders.
To support all Internal teams to follow a mobilisation roadmap so that all new Packages of Care mobilise on their committed start date.
To provide effective rota management of your allocated Packages of Care, ensuring Clients receive high-quality standards of complex care within their homes.
Deliver operational support to the packages within Mobilisation ensuring we meet our company objectives and outcomes.
Have a full understanding of the needs and requirements of your packages for services to be delivered professionally, effectively and within accordance with the care needs.
Liaise with clients, family members, specialist nurses, healthcare and social care professionals, external bodies and all staff within the business, always remaining professional.
Ensure a high level of customer/client satisfaction with excellent and sensitive communication, empathy, the ability to manage complex relationships, proactively managing expectation and creating solutions for clients, providing reassurance and instilling confidence in the service.
Take responsibility for covering unfilled shifts due to sickness or annual leave and update clients and staff accordingly.
Ensure there are sufficient staff available within each package to safely deliver care; regularly assessing the needs of the client and the availability of staff.
Support, Recruitment, onboarding, Clinical and operational teams to manage candidate progression throughout mobilisation
Attend and provide updates on all review meetings with commissioners to monitor outcomes and resolve service delivery issues.
Attend internal meetings as required by the Line Manager.
Travel to regional branches as required for mobilisations.
Qualifications and Skills
Essential
Proven experience In the delivery of complex care / nurse-led care,
Strong understanding of complex care packages, including high-acuity and nurse-led provision
Experience working with ICB or NHS commissioning portals
Demonstrable experience coordinating multiple stakeholders across operational and clinical teams
Highly organised with excellent attention to detail
Strong written communication skills, with the ability to produce clear, compelling responses
Desirable
Experience of mobilisation of complex or specialist care packages
Knowledge of CHC frameworks and NHS commissioning processes
Experience working within a multi-branch or national care organisation
Understanding of workforce planning and capacity management in complex care
