Contract: Permanent, Ful Time, 37.5hrs

Role:
We are looking for a Trusted Assessor to join our team at Cera Care. In this role, you will ensure Care at Home provision is assessed and provided in accordance with the person's identified outcomes, In addition to ensuring provision of support is delivered to maintain the person's independence to continue living at home in local communities. This is an opportunity for someone who has recent senior experience working in healthcare/Social care setting. This role will be based in a branch setting with regular visits in the community.
Key Responsibilities:
Complete Care at Home Assessments for an allocated caseload
Take appropriate steps to safeguard the person as much as possible when care is unavailable.
Plan and ensure packages of care are delivered appropriately, in accordance with the outcome of assessments and the Councils eligibility criteria.
Monitor and ensure the quality and standard of professional practice and performance of service delivery is appropriate and in accordance with the Health & Social Care Standards.
Maintain and update both paper and IT administration systems to promote efficient and effective delivery and monitoring of the service, i.e. use of Microsoft packages, departmental systems and email.
Work with partnership services to meet the needs of service users in the most efficient and appropriate way.
Undertake CPD and participate in corporate staffing initiatives
Requirements:
- Empathetic and dedicated to delivering high-quality care
- Experience working in a healthcare or social care setting, particularly in assessment and discharge planning
- Ability to undertake comprehensive assessments of patients' care needs
- Excellent report-writing and record-keeping skills
- Ability to work independently and make decisions within scope of practice
- Strong communication and interpersonal skills to engage with patients, families, and multi-disciplinary teams
- Strong problem-solving skills and the ability to adapt to changing situations
- Driver with access to own vehicle.
- Ideally NVQ/QCF level 3 in Care.
